called a single-member LLC.okay if you're owned by a single member.and that single member is an individual.you would check off here if you're owned.by a and you would need to change the.name here to ahead and select the.word an LLC and that we are you'll just.say that were a partnership align for.our 2 options to put an exempt payee.code or an exemption from FATCA.reporting code and most of the time.companies leave these blank or.individuals leave these blank this is it.that says that your exempt from backup.withholding so if you read this section.down here on backup of holding and feel.that your exempt from it.then select the number that relates to.you and put that number in the Box a.little further down here if you feel.like you are exempt from facto.withholding sorry from fack up reporting.enter the code here and that's gonna be.a letter so you know if that relates to.you you put a letter a number in here.and you would put a letter in here this.case we're gonna leave it blank most.times this is left blank lines 5 6 & 7.or the address so we'll just key in like.10 Main Street okay this is the this is.the box for your requesters name over.here.most the time again that's left blank as.well but if you want to track who you've.given your w9 to you can key that in.here I suggest leaving this form blank.or this box blank as well as option 7.your for your account number unless.they've required it your requesters.required it that way you can just create.one w9 and you can send that to anybody.that requests it rather than limiting it.to the one person that's requested your.w9 or the one account number that it.relates to all right part one is going.to be your social security number or.your employer identification number so.if you're an individual just keen your.social security number here or if you're.an employer another employer if you are.a company or a entity you can key in an.employer identification number here so.we're just going to say where I an.entity a company we're gonna keep it in.here as a reminder the w-9 form can only.be completed by us persons so a u.s..person would be like a US person a u.s..resident alien or a company
