How do I fill out the IELTS ATRF form for Ryerson University, Canada?

Sorry, cannot help you on this one. I have no idea what the acronyms represent and I graduated in 1971 so forms were paper and filed with the department head. Your best bet would be to go to the RU site and search for these items there.

Do military members have to pay any fee for leave or fiancee forms?

First off there are no fees for leaves or requests for leave in any branch of the United States military. Second there is no such thing as a fiancée form in the U.S. military. There is however a form for applying for a fiancée visa (K-1 Visa)that is available from the Immigration and Customs Service (Fiancé(e) Visas ) which would be processed by the U.S. State Department at a U.S. Consulate or Embassy overseas. However these fiancée visas are for foreigners wishing to enter the United States for the purpose of marriage and are valid for 90 days. They have nothing to do with the military and are Continue Reading

How can I fill out Google's intern host matching form to optimize my chances of receiving a match?

I was selected for a summer internship 2016. I tried to be very open while filling the preference form: I choose many products as my favorite products and I said I'm open about the team I want to join. I even was very open in the location and start date to get host matching interviews (I negotiated the start date in the interview until both me and my host were happy.) You could ask your recruiter to review your form (there are very cool and could help you a lot since they have a bigger experience). Do a search on the potential team. Before the interviews, try to find smart question that you are Continue Reading

How much would I pay for dental filling if I have dental insurance in Canada?

More than what you expect. The reason is because you first need to get an exam and x-rays minimally. And you may be asked to get hygiene done (teeth cleaning) as part of the treatment plan. So before the fillings can be done, expect it to be anywhere from $200–400. You have the co-pay amount that you'd be responsible for, as stipulated by your policy, which can be anywhere from having 50% to 100% coverage. Oh but wait. There's fine print in your policy. Code substitution? They might substitute the code submitted by your dentist and pay for a different code that is less money. Yes they do cleanings, but anything that may take longer than 30 min they will pay at a reduced percentage. Oh, and by the way, they might limit the number of units (amount of time per 15 min blocks) of cleaning you get a year and you can't actually get them to fully pay for a second cleaning in the same calendar year. Fluoride? They may not even pay for it if you're older than 14. Oh. X-rays they impose a yearly limit and they don't pay for both a panorex and full mouth series, just one or the other. If you get a panorex, then they won't pay for any other small x-rays even though they look at different things. Recall exams? 6 months? Read the fine print…it might be 9 or 12 months. Got a specific problem or emergency that you need to go in for an exam? Read the fine print, it might only be covered once a calendar year or if twice for frequency limit, it could not be within half a year. Think you can get all your dental work done with your insurance cuz you'd been waiting to get coverage? Think again. You might have a crappy $1000 limit. $1500 better but not that much better. $2000, $2500…much more leeway to work with. Some are lucky enough to no maximum limit for basic and preventative (though frequentcy limits and code substitutions still apply). Then you can move on to the fillings. It depends on the tooth and the number of surfaces. You are looking at anywhere from $120-$400 a tooth. BC has the lowest fees in Canada so it'd be more in other provinces, especially Alberta. You'd have to calculate your co-pay from that. To know a bit better, you can get a pre-determination from the insurance, but it will delay treatment. You can phone and get some idea. Oh, they don't necessarily honour the pre-determination which is only an estimate. The final adjudication is when the claim is made, when they have all the information about what's paid out and fine print in the policy. Navigating dental insurance is very difficult. Expect to get less than what you think you may be covered for and know that they never cover everything you need, so don't use your coverage limitations as the reason for not getting needed treatment. It will certainly catch up to you in not so good ways.

Why do I have to fill out a W-8BEN form, sent by TD Bank, if I am an F1-student (from Canada) that is not working?

Providing the bank with that firm permits them to apply available exemptions to US withholding taxes. Even if you aren’t working withholding tax might apply to interest earned.

How long do you have to file a dental insurance claim?

As per policy conditions, you might require to do the following : A. To give immediate notice of an accident giving rise to a claim. B. To file a monetary claim and submit the required documents within the stipulated period or as may be fixed by the insurer.

How long do you have to file a claim with Delta Dental?

From tbe FEMA claims manual. The first report of loss, or notice of loss, from the policyholder is the first step in the claims process. The SFIP requires the policyholder to give prompt written notice of loss to the insurer. In addition to the normal information required on the notice of loss, every loss assignment to an adjusting firm should include a brief description of the loss, even if the loss is minor. Information about the loss, such as water depth, affected room areas, and unique circumstances such as accessibility, high valued property, and extent of the damage, is important as it helps to ensure assignment to an adjuster with the appropriate level of experience and will help to prioritize the loss. When the policyholder delays the notice of loss, the adjuster should ask questions so he or she understands the circumstances. The concern here is, if an avoidable delay caused damage to undamaged property, or increased damage from salvageable to non-salvageable property, the SFIP may not provide coverage and the adjuster will face challenges. A justified delay in such instances could be an order issued by local authorities, or prolonged inundation which prevented the policyholder’s prompt access to the insured property. There may be others, but it is important that the adjuster understand the delay and clearly report the explanation.
