that currently1what the child is currently excuse me1that's where you put the address that's1the address to put in this field current1home address and then mailing address1you would fill in if it is any different1from the home address and then rounding1up section one it says here there's a1signature strip here but there's a very1important information that you need to1take care of so here is children 111years of age or over1must assign in this signature box note1at no time should a person other than1the child sign in this signature box1without exception basically meaning if1your child is 11 years old or over they1will sign inside this box of course1beware of this border do not stray1outside of it make sure that your child1signs within it may be a bit more tricky1but maybe have them looking to really1break down a lot of the jargon and some1of the wording here which can lead to a1lot of confusion and make it as1simplistic as possible so hopefully1section 2 I'll clarify a lot of things1for you okay so first of all you're1greeted with some questions here that1you must answer1you cannot leave these blank if you1leave them blank is gonna probably most1probably lead to your application being1rejected or being for sure delayed if if1you need to be contacted for further1information so do not1miss out these sections they're1absolutely pivotal to the child's1passport application so the first1question you agree with is is the child1currently the subject of an adoption1process or partially or fully in the1care of a family services organization1so you must tick no or yes and please be1advised if you take yes then include all1documents pertaining to the adoption1process don't leave anything out keep1just provide oral documentation1pertaining to the adoption process and1then the second question is are there1any separation agreements court orders1or legal proceedings pertaining to1custody of mobility of or access to the1child once again you take either no or1yes and again if it's yes you're going1to provide all of these documentation1that you may have received from the the1courts can vary from country to country1sometimes a real legal agreement may not1be enforced and it might just be between2both parents but court orders of January2issued in most countries and you are2required to provide all documents2pertaining to custody and mobility over2access to the child these are really2important parts of the application you2know if your answer's no don't just2think yeah it's knowing just move on2please tick no or yes and action2accordingly and just below that this is2a section that catches a lot of people2out I think the wording catch a lot of2people out but if you just break it down2it's not too difficult to ascertain and2there is a drop-down list here as well2okay so our relationship between the2applicant2if you really if you get rid of this2bracket and just really like this2relationship between the applicant2parent or legal guardian and the other2parent or legal guardian so it's not2talking about the applicant as in the2child who you were obviously your right2to think is the applicant but when it's2a child it's actually the applicant2parent and then the other parent and2from here if you look at the drop-down2list if you're filling in online it is2much more helpful so we're just talking2about the relationship between the2mother and the father of the child put2it down that way so are they single so2is there no sort of marriage going on2here if there's they've just never been2married and no longer together so it'll2be single so applicant parent isn't no2longer actually in a relationship with2the other parent so then obviously the2applicant parent is single common law so2applicant parent and other parent are in2a relationship with a living at the same2address but they are not married or2haven't put entered any civil2partnerships for example married so2obviously applicant parent and other2parent have gotten married and they've2got the documentation to prove it as2well as the date of marriage if2applicable so if you're married you're2going to be filling in this section with2your marriage date okay then separated2so you put separated here if if the2applicant parent and the other parent2are separated and may potentially be2living apart sometimes you might not2even believe important maybe still be2living in the same address but this2current situation what is what's going2on in the relationship if it's separated2you put that down okay and then divorce2so there's a divorced again you'll still2put date of marriage here in this2section as you would have done with2married and separated if you're2separated but still married it could2obviously be common-law and separates as2volatile on poor divorced just putting2down divorced here and obviously just2the data2marriage will still go here it is still2applicable so don't don't ignore that2just because you're divorced now doesn't2mean obviously weren't married a certain2time so you have to put the date of2marriage there and the finding window so2if one of the parents has passed away2then only that is a status for the2relationship between two parents as of2right now and again if you were married2you will still put the date of marriage2as it's still applicable so again2breaking it down these two sections here2it can get a bit fiddly and it can get a2bit confusing but I'm hoping to really2clarify this section between applicant2parent and other parent and make it a2bit more straightforward process again2what I would think the best thing to do2is get rid of these brackets because2they cause more more of a confusion than2actually helping anything so how you2want to really is applicant parent or2legal guardian but we'll just treat as2applicant parent and then this one other2parent so it's fine so we've got let's2say mum is here and then dad can be here2and then a mum and that can fill in2their information as as they see fit and2and as is correct so for example surname3last name of your birth this is3obviously taking into account that mum3could be here and dad could be here or3dad could be here mum could be here and3what happens is obviously if if a3marriage is occurred and then for3example the husband or wife has changed3the name to the to their spouses surname3it will obviously be different today3name at birth so one of the keys here is3to make sure or if a name has name3changes because maybe if one of the3parents was either adopted after their3birth or maybe they were taken in had a3stepfather stepmother and took on their3name subsequently or and just had a name3change a legal name change so what you3want to do just read this information3really clearly as it states surname3Oh also known as last name at your birth3so it has to be your name at birth or3you know it can also be seen as maiden3names or which is a much more classical3term for that as well but basically you3also name at your birth okay and give a3name so please include all first and3middle names as well don't leave out any3middle names even if you don't use them3day to day life3given names has the S in brackets as3well so it's giving you the space for3that as well of course your date of3birth and your country of birth goes on3here hopefully it has the drop down list3as well so you can just pick the country3of birth and then your current address3here this is the address where you3currently reside so current home address3goes into this section we're both3individuals and then a telephone number3has to be provided as well so daytime3telephone number maybe home cell phone3or work where you can be contacted and I3always recommend always put your email3address in because it's sometimes really3helpful just to get in touch no it's3more of a preferable form of contact3these days with many offices so3definitely provide one if you're maybe3you're just working all day and you3can't see your phone for example an3email might be a know a really great way3for people to get in touch with you or3to trade back and forth documentation3that may be required subsequently and3very finally here for both parents there3will be the signature strip so once3again the signature of the applicant so3again really a signature of applicant3parent okay so that's whoever you decide3to be the applicant parent is on this3side they sign they date and they signed3at Sawle for example signed app would be3Toronto Canada3it could be Ottawa Canada it could be3London United Kingdom it could be Los3Angeles California USA wherever you are3at when you sign this basically we just3want the city and country is what we're3looking for here okay doesn't give you3much space granted but maybe we just3write it on top and then just put it3down with if need be3and the same thing the other parent will3do as well one real big tip I will give3you is as we'll see as we further go3further on into the application onto3page two but one thing I would highly3suggest if if obviously if both parents3are Canadian citizens that both are3Canadian passport holders either one can3be the applicant parent and then the3other one can be the other parent3however I would strongly suggest that3especially if you're applying maybe3outside of Canada where only one of the3parents is a Canadian citizen for3example I would actually recommend that3the non Canadian parent obviously if the3parents are still married or still in a3common-law relationship there's no3separation or anything all three parties3that is the mother the father and the3applicant child are all residing at the3same address then I would recommend that3the non Canadian citizen parent is3actually the applicant parent most3people think that to be the applicant3you have to be the Canadian citizen3parent this is not true basically the4applicant parent is just really filling4in the application form on behalf of the4job because obviously the child is too4young to fill in the application form4for themself and why I'm informing you4of this is it will prove your would much4later on when we get to the guarantor4section as well4for example yep if you are in a4relationship and only one of the parents4is a Canadian citizen have the Canadian4citizen parent become the other parent4okay so a Canadian pair Canadian citizen4is the other parent and then the non4Canadian4be the applicant parent and the our4bills were why and this will save you a4great deal of time and running around4and having to involve third parties it's4gonna make life really really simple4obviously if this is in Canada and both4parents are Canadian citizens either one4can be applicant parent other parent4really a straightforward process but if4it's a situation where only one of the4canady one of the parents is a Canadian4citizen for example through4naturalization and the other parent it4hasn't become Canadian it's boys maybe4just a permanent resident or if you're4applying outside of Canada USA again non4Canadian citizen parent will be the4applicant parent and then the Canadian4citizen parent will be the other parent4on this side4right so we're now at section 3 previous4Canadian travel document so this is a4really straightforward section of the4application so we'll run for it fairly4quickly and basically has a Canadian4travel document passports typical of4identity or Refugee travel document been4issued to a child4we're mainly interested in is passports4so if the child has ever had a Canadian4passport4it could be still valid it could be one4that expired five ten years ago when if4it has been issued it's gonna greatly be4help of help when a processing the4application form so if so it yes and he4is feeling the Canadian passport number4in this field and the date of issue so4this is all available in the bio page4that's the page with the child's photo4on it with all the details so their name4and the day of birth and that's where4you'll find this information so the4passport number which is on top right4hand corner of page three and then the4date of issue which is within the main4body at main body of the bio-data should4be just under the names the order there4and if the Canadian travel document is4still valid you must include it with4your application so if you still have a4valid Canadian passport4you must include it with this4application because that possible would4have to be cancelled and snipped before4a brand new passport can be issued to4you obviously you'll get the the the old4passport back because it asks you about4it would you like the previous possible4return to you if not will be securely4destroyed by the passport program to4protect the child's personal information4so you just hit yes please return it to4me how lucky as a keepsake4and then obviously there's a little bit4of a note here regarding the current4pathway if one was issued previously so4if a valid Canadian travel document has4been lost stolen damaged or is4inaccessible complete from PBT c20 34which is the Declaration connect4concerning a lost stolen inaccessible4damaged or found Canadian travel4document a Canadian travel document once4reported lost hit that and then you would fill in5this section here registration number5so registration number appears on the5Canadian birth certificate the Canary5birth certificate can vary quite a bit5in terms of the sizes depending on which5province has issued it but it's very5very overtly explained listed as a5registration number on the birth5certificate I would recommend whenever5you provide the Canadian birth5certificate5during this processing of your passport5please provide the long form birth5certificate that's the burst of it which5is going to be a bit more larger and5it's gonna have both parents details on5it so that wart just assists in5processing the passport smoothly for5sure you'd have to provide the long-form5birth certificate5the Canadian birth certificate if this5is the first time possible application5but it can always make things a bit more5smoother because5the possible office may not have to then5contact you asking for if they do want5to say it for whatever reason so you put5the registration number here and then5you put the date of issue here birth5registration time and date of issue will5be found on the front of the birth5certificates have a quick scour and5you'll see them clearly defined as they5have issue and registration number on5the birth certificates of the province5where the child was born if the child5was all outside of Canada they would not5be issued of course the Canadian verse5to get there will be of course issued a5birth certificate but won't be a5Canadian one and that's not what you5would use as proof of Canadian5citizenship so the child is born outside5of Canada you of course have to obtain a5certificate of Canadian citizenship5first and only once it's in your5possession are you able to apply for a5Canadian passport for the child so the5Canadian citizenship certificate also5has a certificate number which is very5clearly defined depending on when your5child was born and when the certificate5was issued it would either be a5wallet-sized dual sided laminated card5with the Charles fotona and their name5and date of birth and eye color and5height5and it will have a certificate number in5red and post 2012 those cards have been5replaced and now an a4 cream-colored5dual sided paper certificate is issued5regardless the certificate number is5called the certificate number on both5documents and is very easy to find and5also the date of issue or effective date5of citizenship so the date of issue is5on the pre 2012 citizenship certificates5which are the current citizenship5certificates so if you issued a5citizenship certificate card then the5date of issue5my either just appear as a year for5example 1999 for example or it will be6month and yet it will never be a day6month in the air so you either just have6a year on the front of the card or it6will be oh three 2002 for example so6just put it as verbatim it appears on6the card so if you just get the year you6can just put the year that's fine and an6effective date of citizenship is6regarding the62012 season ship certificate and onwards6the current citizenship certificate6which is again the a4 sheet of paper and6that is visible on actually both front6and back and it is actually called the6effective date of citizenship6section five declaration of applicant6again this section will really trip some6people up because they may have filled6in Section two inaccurately and it just6snowballs from there to make it really6easy the we're gonna scroll up here so6the parent that has filled in this6section here applicant parent side is6going to fill in Section five6declaration of applicant okay so and6this is regarding the applicant parrot6so applicant parent surname goes here6and that's the current surname so for6example if if it's mum she's going to6put a spousal surname if she did indeed6take on the spousal surname for examples6of current surname here the given name6or given names of the applicant6applicant parent here and then6relationship to the child so it's all6going to be mother or father and then6here you put surname of the child okay6surname of the child and then given6names of child it's really important to6read the declaration and know exactly6what you're going to be signing off have6a really detailed read and make sure you6digest all this information and then6applicant parent will be signing here6they will date when they signed here and6then they will put the city and country6they were in when signed6Section six declaration of guarantor so6this can be filled in by a couple of6people when I gave some advice in6section two about making the non6Canadian citizen parent the applicant6and then making the Canadian parent6claim citizen parent the other parent6this is because the other parent can6actually fill in the declaration of6guarantor using a valid Canadian6passport so that's really key and really6that means everything can be done6in-house literally no third parties are6required so for example we don't need to6get a doctor or dentist for example to6fill it in taking the time out of your6day taking time out of their day and6filling it in and having them sign6photos and running around looking for6people when it best suits them this can6be really a simplistic method that a lot6of people kind of rule out or don't even6know about at all6thinking that they're too closer6relation nope if one of the Canadian6parents excuse me one of the parents is6a Canadian citizen and holds about a6Canadian passport they will be the other6parent in Section two and then they fill6in the section six declaration of6guarantor because they are guarantor in6the applicant parent here section five6okay6so what I would do is if you're holding6a valley Canadian pass one that's how6you're going to fill in this application6have your Canadian passport in front of6you and verbatim put in the names as6they appear6in your Canadian passports or same6surnames and given names including6middle names if they appear in your6Canadian passport then here it asks you6for two fields Canadian passport number6so this is where you put your Canadian6passport number and then here I'll ask6you for date of birth in your Canadian6passport not does any passport Canadian6possible okay so you put the Det birth6again verbatim you'll just copy things6over however if neither of the parents6are Canadian passport holders or they6don't just don't hold a Canadian7passport which is currently valid or has7been expired for more than one year you7can get obviously a third party7individual that can either be anyone7else that holds a valid Canadian7passport so you could use siblings of7one of the parents maybe or good friends7of them as long as the guarantor must7know the applicant identified in Section7five okay so is whoever knows this7person obviously if it's apparent holds7about a Canadian past what they're going7to know the other parent as long as I've7known him for two years or more but if7you look for someone that's occupation7base which is another criteria7there we'll fill in a different amount7of information in these sections here so7if it's an occupation based guarantor7and is an exhaustive list here in the7instructions section MTB but basically a7medical doctor a dentist a police7officer pharmacist lawyer for example7those are occupations that are eligible7they were put in here medical doctor7they'll put in lawyer or pharmacist they7wouldn't put their telephone numbers7here as well these keys can be filled in7these telephone fields are going to be7filled in we got irregardless of whether7you're and a Canadian passport holder7guarantor or if you're an occupation7based guarantor those overseas through7the private telephone number because the7guarantor will be contacted to confirm7that they signed the forms and on the7back of the child's photo and then7something that trips up a lot of people7as well especially if they're a7occupation based guarantor they read7this again this is not very well worded7because just this date of birth in7passport really it's only for Canadian7passport holders so if you're an7occupation based guarantor you do not7provide your date of birth you simply7put the name of your firm or7organization organization where you work7so for example if your lawyer7you bought the law firm here if you're a7medical doctor you put the hospital or7the general practice where you're a up7from here or the pharmacists or the7police station where you're prey of if7you're police officer and then you put7again in this address field it's split7so if you're a Canadian passport holder7guarantor you're going to put your7current home address okay and if you're7a occupation based guarantor you will7put your business address7and then finally the guarantor lists the7full name of applicant remember take7away these brackets and make it really7easy so you're going to put the the the7guarantor is going to put the full name7of this person here that's filled in7this section so I don't want the parents7that was the applicant parent they put7their full name here full name of7applicant parent okay and then they put7the full name of the child that is7listed in section 1 then the guarantor7finally will read this declaration7digest and understand what they're7agreeing to what they've signed off and7they will then list that I have known7the applicant parent okay again it's7giving you the clue here is just giving7you this bracket which makes things a7bit more confusing so this is how you7want to read it I hope so I the7guarantor have known the applicant7parent for number of years and the7guarantor has to have known applicant7parent for two years or more in order7for the guarantor to qualify finally the7guarantor or sign here they'll date when7they signed and then they'll put again7the city and country where they were7when they signed7then finally section seven emergency7contact information not the use of the7term optional this does not have to be7filled in for your application to be7successful however I would always advise7providing it especially when it when a7possible application is being lodged in7case of any emergencies that occur7especially when it's become a pertaining8to children it's really handy to have8contact details of someone there's not8normally to travel with the child8because they can really it can really8help out if things go awry out so again8you want to list someone here it's8usually I guess grandparents or aunts or8uncles that don't normally travel with8the child but can be contacted in if in8the worst case scenario and that would8then round out the application process8for the child aboard general Pass8application form but again this can be8used to assist you if you're applying8within Canada or the USA and now some8tips that can be really handy in making8sure that your application is successful8first of all if you are applying for8your child's very first Canadian8passport you have to provide some other8information that isn't really clearly8outlined unfortunately in a lot of8material provided but it's really8important and it can really stop delays8and potential rejections so if it's a8first-time Canadian passport application8you have to provide the original8long-form birth certificate so if it's8the if the jar was born in Canada it8will be the Canadian birth certificate8and that will then8satisfied to documents in one side but8it provides the child's proof of8citizenship of course but also it is a8long-form birth certificate because it8provides the names of both parents on it8if for example the child was born8outside of Canada you would then be8providing two documents one would be the8proof of citizenship so that will be the8citizenship certificate as well as the8original long-form birth certificate so8for example a UK long-form birth8certificate is about a force eyes and is8green and it lists both parents names8and occupations and addresses so that8has to be provided with the first time8child's application form and it has to8be the original documentation and again8if it's a first time application for a8child you also need to provide copies of8valid signature ID for both parents so8this can be for example8valid passports or driving licenses are8the most common choices this is really8just to ensure because it's a first time8Charles possible application there's no8details or ons on record of who are the8parents of the child this is just a8brand-new application and this is just8to make sure that safeguards are in8place that the people that are applying8in section two have the right to be8applying and this works in tandem with8the application form the long-form birth8certificate and then the ID or piece8together information that can then show8that the people were listed on this form8are the parents and they have the right8to apply for the child's applica8passport of course in this day and age8it's it's very welcomed that
