Brooke and I we.lost I'd like to thank first Health.compliance for hosting these webinars.and allowing us to participate Michelle.and I are always thrilled to have an.opportunity to share our experiences on.this exciting topic of credentialing and.yes for us it is credentialing is.exciting although like everyone we've.had our times and we caught it some.other choice words other than exciting.but as dr. Brookes mentioned we're from.company called practice works where we.specialize in assisting healthcare.providers and organizations with.credentialing in provider enrollment.today we're going to cover some of the.broader topics of credentialing in.provider enrollment ima in PES Pecos and.CSU H and the objectives that we want to.try to cover for you today is give you a.better understanding of all these.systems I'll show you how I'm a in PES.Pecos work together we're going to talk.about C aqh and why you need a profile.are your providers need a profile and.then we're going to talk about the.failure or the impact if you fail to.keep that information up to date so.first of all I want to start off with a.poll question I want to find out the.audience's experience with with these.topics so if you could just take a.moment the poll should popped up on your.screen and you could select your your.knowledge level in regards to these.systems.all right excellent so about half of you.are somewhat familiar with these a.quarter of you are very familiar and the.rest of you are having very little to no.familiarity with these things very good.all right so let's just dive right into.it and as dr. Brooks says you have.questions as we go through please feel.free to indicate those and we'll get to.get those questions once we finish each.section so I&A identity in access system.this allows you to access very CMS.computer systems such as npad's and.Pecos to submit updates and their.Medicare enrollment applications it also.grants you access to the EHR Incentive.program for those of you who using.electronic medical health records and I.also allows its users to securely manage.connections between provider.organizations and their relationships at.surrogates you may work on their behalf.we're going to do another quick poll.real quick to find out the level of.understanding or if you even heard of.the IMA system so we're gonna launch.that real quick and see how many of you.familiar with the innate system.excellent so about half of you are.familiar with a little less than half so.we mentioned surrogates.what is the surrogacy program this.program allows your organization to work.on behalf of the decision or the.provider through one account maybe a.practice manager commissioning a contact.at the practice types of accounts that.you can set up our photo provider an.employer so a group on behalf of the.provider or third parties such as.practice or a third party or surrogates.for quite a few practices and there's.different roles for the for this account.being authorized officials for the.practice a delegate officials our staff.and user which I was going to talk a.little bit more about the CFC program.for you now all right so we're going to.talk about why you should use the.surrogacy program one of the biggest.reasons is to remain compliant with CMS.regulations so for those of you that.aren't using the surrogacy program.you're probably getting your providers.username and password for Pecos you're.utilizing that information logging into.their information what CMS would like to.see is you use the surrogacy program so.that the username and password for the.providers are truly their username and.password and protected by them where.we're using the surrogacy account so.remain in compliance with CMS the other.reasons are you're going to have easier.access to your provider account and.information so with that being said.probably the one of the most challenging.the things to do when you're trying to.enroll a provider is figure out have.they ever been enrolled with Pecos do.they have an active enrollment are they.deactivated what's their status by being.able to log in through the surrogacy.program you can access your provider and.find out what their current enrollment.status is and the other reason would be.faster enrollment and turnaround time so.it is proven that by using panko your.enrollment can be completed with within.60 to 90 days where a paper application.can take anywhere from 120 days to 150.depending upon your map.in the state you're located in how you.register is fairly simple a providers.automatically enrolled when they apply.for an NPI number so that username and.password is what they are going to use.to access and create their account and.for surrogates such as ourselves.practice works is an active surrogate.you will log in using the website listed.below to go and create your own account.so next we're going to show you what the.identity and access system looks like if.you're not familiar this is the main.screen when you go to access the website.you're always going to select accept and.then you're going to get into the.sign-in screen so if you do not have an.account you're going to access the right.side of the screen where it says create.an account and you will then be prompted.to enter an email address and you'll go.through the steps to create a password.one thing I want to point out here is.you cannot utilize your email address.more than one time so once you create an.account if you were to leave company ABC.and go work for company XYZ this account.is going to follow you wherever you go.so it's very important that you remember.the information and it's very important.to realize you can only have one email.address so you would set it up here.submit and then your account would be.created now what you're seeing is we.already have an active account as.practice work and we're showing you how.you can add a provider and be a.surrogate on their behalf so once you.log in you'll see a home tab on my.profile my connections and my staff.we're currently on the my connections.tab and you would select the yellow.button at the bottom that states five.providers then to search for a provider.and/or organization you're going to be.prompted to enter the following.information so you can either search by.the organization name provider last name.city state and zip or the NPI you only.enter one way or the other you never.enter both the easiest ways to search.mpi number once you select and pull up.the provider and/or organization you're.looking for their name will appear.across the top then you will have in the.middle section the ability to select.what you want access to so here you can.select Pecos the EHR Incentive program.or in DES.once you select those you'll hit.continue then it is going to allow you.to review what you've selected so here.in stating that we have requested access.to Pecos and in PES it's going to.request if you need an additional email.address so if your provider doesn't.utilize the initial email address that's.on their original account in the.additional email address field you can.enter an alternative email address to.ensure they receive that notification.you will then hit submit and then be.prompted for one more review this is.just letting you know that you have.successfully submitted a connection.request to work on the provider's behalf.the details are listed below it will.give you a tracking ID and you will then.select done once you complete that field.then the provider would log into their.own account they will then see the.request on their home screen and they.can then select to approve you as a.surrogate or decline and now I'm going.to turn it back over to Josh she's going.to get into some information on input.all right while we do that we have.another poll so we're going to find out.how many of you actually use the.security program so if you take a minute.vote on that one.then close the poll and share the.results here.so a little more 2/3 of everyone here.does not use the sexy program so that's.I think that's pretty standard across.the board as far as he's using the.program or not all right so now we're.going to get in to impasse sure.everyone's familiar with impediments.spend too much time on it so impasse.it's a national plan and provider.enumeration system this is how.individuals and organizations apply for.NPI numbers and as sure everyone that.was mpi numbers are the national.provider identifier is used for.contracting credentialing and billing.purposes to associate the provider with.an organizational health plan now.obviously we start with ima first intent.is what actually kicked this whole.process office is applying for the NPI.number when you do apply for your NPI.number you're going to visit their.website which is another previous slide.and this is how you create user name and.password and that's what sticks with a.provider throughout as far as their I&A.system and Pecos EHR as we discussed a.little more so on this screen this is.the home screen for the NPI registry.this is where you will create an NPI for.an indoor provider organization that's.also when you'll access this if you need.to do any updates for that so a few.little key items as far as n says goes.once an NPI number is established it.stays with the provider organization it.never changes the information and in.pads is self-reported so the provider.practice manager whoever setting up the.NPI is the one who provide that.information and that's why it's.important we're to always make update.cuz it is self-reported things that.should be updated change of address.practice location residential.information tax on amico is very.important especially for billing.purposes the contact person make sure.that's current and up-to-date as far as.contact information email address and.then adding.modifiers such as your license or your.medicare/medicaid numbers in fads is.regulated by CMS.so all changes according to CMS policy.should be updated within sixty days of.that change in fads information can also.be managed through Pecos when you're.submitting an online provider enrollment.or reassignment application you can also.make updates to your inset account that.time searching someone's NPI number can.also be done to the NPI registry it.would be the screen where you can in.that information now if someone forgets.their username and password which does.happen quite frequently there is a paper.form that can be submitted to CMS form.that can be submitted to update that.information this form is available.through the CMS website original.signature so the original form must be.submitted from any changes to view all.right now Michelle is going to get into.Pecos alright so now we are going to.talk about Pecos which is the provider.enrollment chain and ownership system.and we're going to talk about why we.should use it so we talked a little bit.earlier about the benefits of the.surrogacy program in acting as a.surrogate on behalf of a provider the.advantages that have but now we're going.to talk about Pecos and and Wyatt you.should use Pecos over a paper.application so the number one reason.people utilize makos and what we.encourage all of our clients why we.would like to use papers for them is it.will significantly decrease the amount.of time it takes to process a medicare.application so as I mentioned earlier.there's processing timeframes.Pecos you're looking at 60 to 90 days a.paper application is typically 120 plus.days depending upon your Mac easier.access to provider information so again.rather than trying to figure out you.know a paper application am I submitting.in eight five five I for an individual.and an eight five five are you can log.into Pecos and figure out what is their.status.what stared and rolled with if they're.moving from one state to another.are they current are they to be.activated do we need to revalidate and.you also have the ability to verify.reassignments and effective dates the.reassignment becomes a big deal when you.have providers leaving organizations.maybe they're coming to only work for.your organization but they're still.actively enrolled with several other.groups if the other organization has not.terminated that affiliation you will be.able to do so by viewing this.information and reviewing it with your.provider asking them if they should be.terminated if they're to be active and.that's something that you know it's good.to know as well so you can communicate.with that organization on correspondence.because it will not always go to you if.there's more than one reassignment on.file and finally you have the ability to.electronically sign you no longer have.to submit a paper original signature to.Medicare if you utilize the Pecos system.you can't electronically sign and how.that work you'll type in the provider of.email address they will get an email.they open the link they enter their name.date of birth social and they'll be.given a unique PIN number and they can.electronically find that application.this is the home screen for those of you.not familiar with Pecos when you log in.so you'll enter the username and.password here now if you are using the.surrogacy program your individual user.name and password would be entered here.you would select login and once you log.in this is going to be your home screen.so just a few things I want to point out.here the my associates tab is where you.can generate a new application perform a.change of information so then a.revalidation and manage and see exactly.how the provider is currently enrolled.in and what's going on I also want to.point out the revalidation notification.center that's something new and we'll go.onto the next slide.the revalidate validation notification.center is.something that CMS has created and it's.available through Pecos we are actually.looking at my surrogacy account so the.advantage of this is that I am able to.see every provider that I am a surrogate.for the type of provider they are the.type of application their enrollment.status and their revalidation due date.so if any of my providers are due for.revalidation or have passed revalidation.for one reason or another I'm able to.see it at a quick snapshot here talking.about revalidation I want to spend a.little time on cycle 2 revalidation so.cycle to Ravel to place in late 2015.your notices will begin going out to you.6 months prior to the due date so the.most important thing that I want.everyone to realize with cycle 2 and if.you are incurring this problem you've.already experienced it cycle 1 if you.missed your due date you didn't have a.gap in billing you simply completed your.application and then your effective date.went back to your deactivation date you.didn't have a gap in billing the biggest.change with cycle 2 is now if you miss.your date you will be deactivated and.you are going to have a gap in billing.so if my provider is being activated.today November 15 there will be a gap in.billing until I get him approved and.revalidated some resources that CMS has.provided to ensure you can stay on top.of this and your providers are not.deactivated we have a link here that.godot CMS gov Medicare validation few.visit that website they have an online.that's basically an Excel spreadsheet.you can download that report you can.access the revalidation Notification.Center through Pecos which we just.viewed look at your individual providers.and/or those that you're a surrogate for.and finally CMS will send a.correspondence letter just something you.should note though if the provider is.reassigned of more than one group you.may not be.one that gets the letter so that's where.very important if the provider is.actively still working for one or two.organizations develop a relationship.with them so that you are all you know.discussing the Pecos and what's going on.because they may get something that.applies to you and vice versa and just.some reminders revalidation is due every.three years for DME and every five years.for all other provider types and when.you're revalidating be sure to.revalidate all information on the.application including changes in address.authorized official update or license.your contact person etc the other topic.that we're going to spend some time on.today regarding Pecos and Medicare.enrollment in general is revocation so.CMS may revoke and enroll providers.suppliers Medicare billing privileges.for any of the following reasons some of.these are going to spend more time on.others but the first one is.non-compliance with the enrollment.requirements described in the enrollment.application and if you're found to be.non-compliant and you have not submitted.a plan of corrective action you will be.revoked.so we've referenced some CMS manuals in.here throughout these slides I would.encourage you to go look at those we're.not going to review those in detail but.they will give you more information.another reason revocation occurs is if.the providers suppliers conduct which.includes any owner managing employee or.authorized official felony.so providers supplier owner managing.employee authorized official if any of.them were convicted within the preceding.10 years your organization could be.revoked number for false or misleading.information on the application that.asserted certified is true so I am going.to give you an example of something we.encounter here with one of our clients.we had an organization that had a.authorized official listed as 5% or more.ownership.a nonprofit organization therefore they.didn't have any ownership CMS requested.a fingerprint background screening on.them this person did not want to comply.with that because they were not a true.owner so while this was discovered we.essentially were helping clean this up.we inherited inherited the mistakes we.submitted the application to correct to.change this person from the owner just.to authorize official but since CMS.already requested the background.screening and they did not comply we.then got a revocation for this.particular state and all organizations.and locations within that state we then.took that revocation and fortunately has.some contacts at CMS we were able to.reach out to explain the situation they.were able to find the application we.submitted for correction it was in.process they fixed it and within 24.hours the revocation was fixed back to.active now we were lucky in the sense.that again we did have some contacts at.CMS we were aware of this and it turned.out to be okay however had that not.happened we would have had to go through.a very very very long extensive process.to get them reactivated for not having.correct information on the application.so always make sure that the information.you're submitting on these applications.is true and correct if you're not sure.don't submit the application reach out.to however you need to to ensure that.you have all of the facts as true the.next reason would be on site review if a.pond and site visit CMS determines one.of the following no longer operational.this is another one we encounter a lot.with our clients operational you need to.refer to CMS's definition of operational.but that means that you are up and.running your operational you have beds.in your office you have staff actively.working you have all of the correct sign.is posted we have a lot of.that want to open a brand-new practice.with no staff no signage and a CMS comes.out there and does a site visit and you.don't look to be operational you will be.revoked so make sure you understand the.definition of operational when you need.an on-site review another reason would.be grounds related to screening.requirements misuse of billing numbers.abusive billing privileges failure to.report any major changes including.ownership change of address etc failure.to document and provide CMS access to.documentation and if your terminated.from Medicaid you're going to get an.automatic revocation with Pecos so what.happens if revocation occurs so you know.we talked a little bit about our example.our experience you know again that was.that was just pure luck that doesn't.happen all the time but if you were to.be revoked the provider supplier must.wait a minimum of one to three years.depending upon the severity of the.reason for revocation before Rhian.rolling once the providers suppliers.revoked you must submit a brand-new.enrollment application and a new site.survey or certification is required so.you will have to go through that process.again and revocation becomes effective.30 days after CMS mails notice to the.provider or supplier notifying them that.you're going to be revoked so if you get.a revocation and you're trying to get a.reversal if the revocation is due to an.adverse action by an owner managing.employee or authorized official it may.be reversed within 30 days if the.provider and supplier submits proof the.relationship with the offending.individual has been terminated so if.your revocation is due to something any.one of these members in your.organization have done something you can.prove that you have since terminated.them it can be reversed within 30 days.if you do have more questions on.revocation or cycle to revalidation.please let us know or you can reach out.to Josh and I.their words and we'll be happy to go.into more details or some case-by-case.scenarios for you and right now though I.think josh is going to talk a little bit.about PA qh4 you all right I have one.last poll question for us you want to.find out how many of you actually use.Pecos or do you still use paper so if.you take a minute and answer that.appreciate it.all right go ahead and close the poll.and it's like the majority of everyone.is using fake codes so that's excellent.does save a lot of time okay so our last.topic it's gonna be C aqh Council for.affordable quality health care c AQ h.was an initiative kind of spearheaded by.a lot of payers to have a self-reported.provider database to help streamline.some of the credentialing and.maintaining credentials for all their.network providers sure everyone is.somewhat familiar to seeing a qh this is.the home screen for the CH pro view.which in which everyone would use to set.up a profile so why you see a qh main.reason is participating payers can.access this provider information and it.helps to reduce the amount of paper.applications that need to be completed.to simplify the credentialing enrollment.and should save a little bit of time for.enrollment it ensures accuracy with.information the payers have and that you.have entered into that and from.applications because you're only.entering the information one time and.it's a nice database that you can access.and provider database to see a QA to.keep this information in one location.and also include supporting.documentation lights copies of license.practice insurance and various other.documents that CA th requires a couple.key items notes the aqh it must be.retested every 120 days this lets the.payers know that information has been.viewed it's not stale if the if it's not.reassessed that's the aqh considers the.profile and information in that profile.be outdated and then the payers won't.use that profile won't use that.information for credentialing or.reproduction if it's not updated and.hasn't been reattached.ch-ch-ch is free and there's a practice.manager module that allows the.administrator or the commission contact.for organizations to set up and link all.the existing providers.organization under the practice managers.profile or account this allows for some.bulk uploads for information for.instance if practice has multiple.practice locations if there's any.changes to the address contact number.contact person credentialing person.amount practice information for the.group of the whole this can be uploaded.on a bulk basis to provider management.account can save a lot of time.especially for large organizations with.multiple providers that information can.be entered once to the practice.administrative practice managers account.and then upload it to each each of.providers and then you would just log.into the provider account to accept that.change of information saves a lot of.time by using that that account so again.as we discussed before what happens this.provider doesn't know his username or.password I'm sure we've all encountered.that a time or two so a couple of oxygen.options you can have a provider call in.to see a qh2 request this information.not always practical doctor it's always.busy doesn't have time to call in the.other thing you can use that it's not.published on the tqh website but they.have a they have an authorization and.release form I'll show that to you on.the next slide but just want to kind of.go to the information that's required.obviously the providers see you hid.their name and then if they have a new.primary email address associated with.their account as well as copies of state.license that are on file with the aqh.these things are going to be need to.accompany this form once it's completed.and it can be emailed to the provider.help reprove you at CA qh as well this.form is the authorization and release of.information to designated contacts so.what this form does it basically grants.you as the credentialing contact for the.provider information are these access to.the providers account to update that.information this form like I said is not.published on this da QH website so it is.something that you want you can contact.from seller I we can have dr. Brooks to.see if they wanted to post that as well.but it is available to use.as another option for updating CH.football and that brings us to a close.here so if any do you have any questions.that you'd like to write in now or feel.free to contact us offline we'd be happy.to answer and help you that that's what.we can Thank You Josh Thank You Michelle.we'll wait and see if there are any.other questions we do have one in the.meantime how long does it take to enroll.a provider well that's the most common.question we get from our clients and it.varies actually it really varies upon a.lot of different factors could be the.particular insurer that's right and roll.with whether it's a new grad or an.experienced provider is someone joining.an existing practice or starting a new.practice even the time of the year can.determine how long it's going to take.but but on average you know we do have.some averages based upon our experience.or new grad enrolls Medicare to be.ninety two hundred days is what we.usually allow for enrollment through.Pecos an existing provider who's.reassigning benefits to an existing.group average is about 75 days although.with certain max can take you know we've.seen it take you know 45 days even a.little bit less than that commercial.insurers have a wider range you know.we've had providers joining a new group.where it only takes 30 days to get to.get credentialed enrolled and links to.that group and you know with a new.provider with a new group you know 150.280 days we've seen it take so it really.is a wide range based upon you know the.various factors and we have one more.question is it necessary or beneficial.for providers to be enrolled in both.Pecos and C aqh yes absolutely.so Pecos Pecos is what's governing CMS.for Medicare so you have to have that.Pecos enrollment for Medicare see aqh is.more for commercial insurers so that's.obviously if you going to enroll with.either one then you have to have both.profiles if you only have a Medicare or.Medicaid.hi practice ourself you know cash based.practice you know then you wouldn't need.CH you H but you do need both you know.because they have the different factors.Medicare beam for Pecos and commercial.insurers you see a few H all right and.do I enroll as a group or as an.individual well it really depends if.you're new practice we we do get that.quite a bit quite often some new new.provider setting up a new practice we.always recommend if there's any chance.down the road that you're gonna have a.new associate maybe a partnership with.someone else to go ahead and set up your.group and then enroll as a group with.all the payers you know set up a.Medicare Group group number and as well.as get a group contract within.commercial payers so but it's really a.personal choice if you want you know if.you know you're always going to be solo.then you can remain individual but if.there's any chance down the road of you.incorporating someone else into your.practice and it's just easier if you do.it upfront than to go back down the road.and do that okay.and when C aqh sends the approval email.for example certain insurances does this.mean the provider is good to go with.that insurance or do we need to wait.till we receive notification from that.insurance carrier so with that one when.you get notification from CA qh i just.want to clarify you you'll get two.emails so the one email is that you may.get an email that the payer has added.this provider to the roster or through.CA th and another email may just be that.the application is successful and.they're good to go.you always need to wait until you get.the actual approval from the health plan.and/or payer for your approval see aqh.doesn't really determine that it's.simply a tool that is used to help you.with enrollment and make that process.more efficient.okay I think that wraps up for the.questions currently thank you again Josh.Michelle a great discussion please use.their contact information for questions.or if you send them to us we will.forward them on to them please join us.again at noon Eastern Standard Time on.December 13th for is a firewall enough.security for your business with Albert.wale of IT security incorporated and.then again on the 14th of December for.2017 procedure codes update a-to-z with.karna mora of coding strategies you can.register for these webinars and request.a demo of our compliance solution on our.website one st HCC comm or email us at.info at one st hcc comm or call eight.eight eight five four three four seven.seven eight thank you very much and have.a great day.
